Report your Outcomes, keep Police informed

Assessing the effectiveness of the Police Referrals is something that is important to all involved. Like any program, having visibility over what is working and the impacts of the work being delivered needs to be shared in order to streamline processes. In order to capture this information we developed the Outcomes Component which is an effective and easily assessable tool contained within the Police Referral System. The tool should be part of the natural flow-on process of accessing and actioning referrals and can be completed after closing referrals with a Service Provider Action as “Client Contacted – Services Accepted OR On-referred.”

Update Referrals with Actions

As part of keeping police informed we would ask that you always update a referral with a Service Provider Action at any time in the life of a referral. This would be in instances where referred clients have contacted your service at a later date and have engaged with your service or program, please update the referral to reflect this.
